What to Know First

Exercise can be an important part of living well with lymphedema. Walking, strength training, swimming, cycling, stretching, and other forms of movement can support overall health while helping activate the muscles that assist lymphatic and venous circulation.

But if you normally wear compression, an obvious question comes up:

Should you keep your compression garment on while you exercise?

For many people with established lymphedema, the answer is yes—compression can often be worn during exercise and may be recommended as part of an individualized lymphedema management plan.

That does not mean every person needs compression during every workout.

The right answer depends on your type of lymphedema, the garment you wear, the activity you are doing, your current symptoms, and the recommendations of your healthcare provider or Certified Lymphedema Therapist (CLT).

Here’s what to consider before your next workout.

Why Exercise Is Important for People With Lymphedema

Your lymphatic system does not have a central pump like the heart.

Instead, lymph movement is assisted by several mechanisms, including muscle contractions, breathing, body movement, and changes in pressure within tissues.

When your muscles contract during exercise, they create a pumping action that can support movement of fluid through the venous and lymphatic systems.

Exercise can also provide benefits that extend far beyond swelling management, including improved:

Cardiovascular fitness

Muscle strength

Joint mobility

Balance

Functional ability

Bone health

Weight management

Overall quality of life

Current clinical guidance supports appropriately prescribed aerobic and resistance exercise for people who have or are at risk for lymphedema. Exercise programs should generally be individualized, introduced gradually, and monitored for changes in symptoms.

Importantly, the older idea that people with lymphedema need to avoid using an affected arm or leg has not held up well under modern research.

Progressive resistance training, for example, has repeatedly been shown to be feasible and safe for many people with breast cancer-related lymphedema when appropriately prescribed and progressed.

So, Should You Wear Compression While Exercising?

There isn’t one universal rule.

For people with confirmed lymphedema, many lymphedema specialists recommend wearing properly fitted compression during aerobic or resistance exercise, especially when that compression is already part of the person’s daily management program.

Compression combined with muscle activity may help support fluid movement and provide containment while the limb is working.

Compression is also incorporated into Complete Decongestive Therapy, where movement and exercise are frequently performed while the affected area is compressed.

However, research has not shown that everyone with lymphedema will experience a flare simply because they exercise without a garment.

Studies comparing exercise with and without compression have produced more nuanced findings. Some research in breast cancer-related lymphedema has found no significant short-term difference in limb volume between resistance exercise performed with compression and the same exercise performed without it.

A more recent study involving people with cancer-related lower-limb lymphedema similarly found that a single session of high-intensity interval cycling appeared safe both with and without compression. That does not mean compression is unnecessary; rather, it reinforces the importance of individualized recommendations.

Compression and Exercise Can Work Together

It can be useful to think about exercise and compression as two different tools that may complement each other.

Exercise provides active muscle pumping.

Compression provides external support and containment.

When a muscle contracts beneath an appropriately fitted compression garment, pressure within the tissues changes. This combination may help support venous and lymphatic fluid movement.

That is one reason exercise is frequently incorporated into compression-based lymphedema management.

But the key words are appropriately fitted.

Exercising in a garment that is too tight, damaged, folded, rolling, or poorly positioned is very different from exercising in compression that fits correctly.

What Kind of Compression Can Be Worn During Exercise?

The best option depends on where you have swelling and what has been recommended for you.

Compression Arm Sleeves

People with upper-extremity lymphedema may exercise in a properly fitted compression arm sleeve.

Depending on the distribution of swelling, the sleeve may be paired with a compression glove or gauntlet.

This can be particularly relevant for activities involving substantial arm movement, such as:

Resistance training

Rowing

Walking with poles

Racquet sports

Fitness classes

Gardening

Repetitive occupational activity

If you experience hand swelling, wearing a sleeve without appropriate hand compression may not adequately address your individual swelling pattern. Ask your lymphedema therapist whether a glove or gauntlet should accompany your sleeve.

Compression Stockings

For lower-extremity lymphedema, properly fitted knee-high, thigh-high, or full-leg compression may be worn during activities such as walking, cycling, strength training, or other land-based exercise.

Movement of the ankle and calf muscles creates an especially important pumping action during walking.

If you already wear daytime compression stockings while you are active, your clinician may recommend keeping them on during many types of exercise as well.

Adjustable Compression Wraps

Some people manage lymphedema with adjustable compression wraps rather than traditional stockings or sleeves.

These products use overlapping straps that allow compression to be adjusted.

Depending on the wrap and your activity, an adjustable system may be appropriate during certain forms of movement or therapeutic exercise.

One advantage is that the wrap can often be readjusted if limb volume changes.

However, bulkier wraps may not be practical for every sport or piece of exercise equipment.

Footwear fit is especially important when exercising in a lower-leg or foot wrap.

What About Strength Training?

Strength training was once approached very cautiously for people at risk for or living with lymphedema.

Today, the evidence is much more encouraging.

Progressive resistance exercise can generally be performed safely by many people with lymphedema when the program is introduced appropriately.

The emphasis should be on progressive.

That means starting at a manageable level and increasing resistance gradually rather than suddenly attempting heavy loads your body is not accustomed to.

For example, someone returning to resistance training might begin with relatively light resistance and focus on:

<img data-recalc-dims="1" progression

Adequate recovery

Monitoring the affected limb

The goal is not to avoid challenging your muscles forever.

It is to give your body time to adapt.

People who have undergone cancer surgery, radiation, lymph node removal, or other procedures may also have shoulder limitations, scar restrictions, weakness, balance changes, neuropathy, or other considerations unrelated to lymphedema itself.

A physical therapist, occupational therapist, CLT, or appropriately qualified exercise professional can help modify a program when needed.

Can You Walk or Run in Compression?

Walking is one of the simplest ways to combine muscle activity with compression.

With each step, the calf muscles contract and relax, contributing to the calf-muscle pump.

For someone with lower-extremity lymphedema who normally wears daytime compression, walking in their prescribed compression is often a natural extension of their daily routine.

Running can also be appropriate for some people.

However, higher-impact exercise introduces additional considerations:

<img progressively rather than jumping suddenly from low activity to long or demanding workouts.

What About Cycling?

Cycling can provide repetitive lower-extremity muscle activity without the same impact forces as running.

People with lower-extremity lymphedema may be able to cycle while wearing compression stockings or another prescribed compression system.

Pay particular attention to areas where clothing, shoes, straps, or the bicycle itself may create additional pressure.

A 2025 randomized crossover trial involving people with cancer-related lower-limb lymphedema evaluated high-intensity interval cycling with and without compression. Researchers reported no adverse events and no clinically meaningful difference in limb volume or self-reported symptoms between the conditions, providing additional evidence that exercise can be safely individualized.

Do You Need Compression While Swimming?

Swimming and aquatic exercise are a special case.

Water naturally creates external pressure on the body. The deeper the body is submerged, the greater the hydrostatic pressure.

<img data-recalc-dims="1" generally do not simply wear their regular compression stocking or sleeve in the pool unless the garment is specifically intended for that purpose or their clinician has recommended it.

Standard compression garments may also be affected by pool chemicals, salt water, repeated saturation, or stretching.

If aquatic exercise is part of your routine, ask your therapist whether you should exercise without compression in the water and when your normal garment should be reapplied afterward.

Heat Changes the Equation

Exercise generates heat.

Compression garments can also make the skin feel warmer, particularly in hot or humid environments.

<img data-recalc-dims="1" loading="lazy" decoding="async" width="374" individuals.

That does not necessarily mean you need to stop exercising during warm weather.

Instead, consider adjusting your routine.

Exercise during cooler parts of the day, choose climate-controlled environments when possible, stay hydrated, take breaks, and monitor how your limb responds.

Moisture-wicking clothing can also make exercise more comfortable around compression garments.

After a sweaty workout, do not leave a damp compression garment against your skin indefinitely.

Skin and garment care become class="wp-block-paragraph">Skin protection matters because damage to the skin can create an entry point for bacteria.

Wash and dry the skin appropriately after sweating and follow the skin-care routine recommended by your clinician.

<h2 it is time to have your compression garment reassessed.

A garment should not need constant tugging and repositioning throughout your workout.

Don’t Make a Too-Tight Garment Your “Workout Garment”

More compression is not automatically better.

You should not purposely wear an undersized garment because you assume harder exercise requires stronger compression.

Compression class, fabric, garment style, and fit should be selected according to your individual needs.

Excessive or poorly distributed pressure may create discomfort and can be inappropriate for people with certain vascular or medical conditions.

Similarly, doubling garments or adding extra compression without professional guidance is not a good way to improvise a workout system.

When You Should Pause Exercise

Exercise is generally beneficial, but new symptoms should not be ignored simply because they appear during a workout.

Stop exercising and seek appropriate medical or clinical guidance if you develop concerning symptoms such as a sudden unexplained increase in swelling, a red and unusually warm limb, severe or unusual pain, new numbness, significant skin color changes, chest pain, unusual shortness of breath, or symptoms suggesting an acute infection or vascular problem.

If your lymphedema is normally stable and suddenly behaves very differently, don’t simply increase compression and push through the workout.

A significant change deserves evaluation.

How to Start Exercising With Lymphedema

You do not need to build the perfect fitness program on your first day.

Start with an activity you can perform consistently.

That might be a short walk, gentle stationary cycling, a few resistance exercises, or a therapist-prescribed movement routine.

Then progress gradually.

Pay attention not only to how the limb feels during exercise, but also several hours later and the following day.

<figure its normal baseline after recovery?

These observations can help you and your clinician determine whether your program or compression strategy should be adjusted.

Exercise at Risk of Lymphedema vs. Exercise With Lymphedema

There is also an important distinction between someone who has lymphedema and someone who is simply at risk of developing it.

For example, a person who has undergone lymph node removal after cancer surgery may be considered at increased risk but may never develop clinically significant swelling.

People at risk are not automatically required to purchase and wear compression every time they exercise.

Current evidence supports progressive exercise for people at risk for breast cancer-related lymphedema, and appropriately progressed resistance training has been shown to be safe.

If you are at risk but do not currently have lymphedema, discuss whether preventive compression is appropriate for your individual situation rather than assuming it is mandatory.

Your Compression Should Match Your Activity

You may eventually discover that different activities work better with different compression options.

For example, someone might prefer:

<img data-recalc-dims="1" loading="lazy" decoding="async" width="374" height="328" has-alpha-channel-opacity"/>

The Bottom Line: Compression and Exercise Should Be Individualized

So, can you wear compression garments while exercising?

In many cases, absolutely.

Exercise and compression are commonly used together in lymphedema management, and people with lymphedema should not automatically avoid aerobic activity or resistance training because they are concerned that movement will worsen swelling.

At the same time, current evidence does not support a rigid rule that every person with lymphedema must wear compression during every possible form of exercise.

<img progression, good skin care, appropriate hydration, and awareness of changes in swelling or symptoms can make physical activity more comfortable and sustainable.

Most importantly, your compression should support your ability to move—not prevent it.

If you are beginning a new exercise program, returning after a long period of inactivity, experiencing a change in swelling, or unsure whether your current garment is appropriate for exercise, discuss your plans with a Certified Lymphedema Therapist or another healthcare professional familiar with your medical history.

<h2 The evidence does not support an absolute rule for every person and every activity. Research comparing exercise with and without compression has not consistently shown that exercising without a garment causes an immediate worsening of lymphedema. However, your decision should reflect your diagnosis, swelling pattern, activity, and clinician’s recommendations.

</details>

<details class="wp-block-themeisle-blocks-accordion-item">

Can I swim with lymphedema?

Aquatic exercise can be a useful option for many people with lymphedema. Water provides hydrostatic pressure around the body, and a standard daytime compression garment is generally not simply worn in the pool unless specifically recommended or designed for aquatic use.

</details>

<details class="wp-block-themeisle-blocks-accordion-item">

Should I wear compression during yoga or stretching?

There is less research specifically addressing compression during flexibility-only activities.
